LPN: Licensed Practical Nurse Job Summary

We are seeking a compassionate and skilled LPN to join our team at Three Pillars Senior Living Communities. As an LPN, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities:
  • Administer medications and treatments as ordered by the physician and in accordance with accepted standards of practice and Three Pillars policies and procedures.
  • Maintain accurate and timely written and electronic charts/documents/records as required by law, regulations, and Three Pillars policies and procedures.
  • Observe and record resident condition information, reporting findings to and collaborating with RN to develop and implement plan of care for the resident.
  • Communicate resident change of condition to appropriate persons, including nurse manager/supervisor, RN, physician, family, clergy, hospice team, and interdisciplinary team.
  • Assist in the admission/readmission process as directed by the RN, including observing and gathering of clinical data and development and implementation of the initial plan of care.
  • Obtain, transcribe, and implement physician's orders according to accepted standards of practice and Three Pillars policies and procedures.
  • Support and participate in all resident care programs, including following individual POCs, documentation, communication of observations, and offering of feedback/suggestions.
Requirements:
  • Minimum education, certification, and/or licensure: Licensed Practical Nurse in Wisconsin.
  • Current and valid CPR certification in long term care preferred.
  • Basic computer knowledge.
  • Able to frequently push/pull objects weighing 60 pounds and occasionally lift and move objects weighing 100 pounds or more.
