Day Programs Care Coordinator

About the RoleWe are seeking a highly motivated Day Programs Direct Support Professional to join our team As a key member of our staff, you will play a vital role in facilitating, modeling, and building skills that lead to optimal independence for our participants.
Responsibilities- Support and Assistance: Deliver training, support, and assistance to participants in areas such as personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising individual rights, community integration, and behavior management.
- Individualized Support Plans: Participate in the design, implementation, and monitoring of participants' individualized support plans, ensuring your observations and opinions contribute to tailored, achievable goals.
- Medication Administration: Accurately administer prescribed medications to participants as ordered by physicians.
- Documentation and Reporting: Complete required documentation, including program data, incident reports, daily logs, progress notes, and health records, accurately and efficiently.
- Health and Safety: Ensure participants' health and safety, preventing abuse, performing CPR, Standard First Aid, Heimlich procedure, and evacuating participants in case of a fire.
- Transportation and Meal Preparation: Provide transportation to recreational and community activities and prepare participants' meals following published menus.
- Prior Experience: Prior experience working with people who have developmental and physical disabilities is preferred.
- Life Skills Mentorship: Ability to mentor and assist people in all areas of life skills, including recreation, independence, communication, and health.
- Valid Driver's License: Valid driver's license required.
- Education: High School Diploma or GED Required.
- Communication Skills: Basic computer aptitude and ability to speak/write in English.
