Job Summary: Veterans Inc. is seeking a highly motivated and skilled Case Manager to join our team. As a Rapid Rehousing and Homeless Prevention Case Manager, you will be responsible for delivering supportive services to veterans in need of homelessness prevention and rapid rehousing assistance.

Key Responsibilities:

  • Perform initial intake and assessments of veterans for acceptance into Veterans Inc. programs.
  • Develop individualized service plans for homelessness prevention and rapid rehousing based on veterans' input, interests, and needs.
  • Coordinate veterans' participation in agency programs and assist with researching and accessing needed services.
  • Track and report case management progress through case notes and conferences with Program Manager.

Requirements:

  • Bachelor's degree in Human Services, Social Work, or related field.
  • Minimum of two years' experience in Employment/Training, Vocational Rehabilitation, Counseling, Outreach, or related field.
  • Computer proficiency in Microsoft Office Suite.
  • Ability to pass a CORI and background investigation.
