At LHC Group, we are seeking a highly skilled Clinical Rehabilitation Therapist to join our team in Southport, Connecticut.

About the Role

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. They will work collaboratively with the healthcare team to develop and implement treatment plans, provide interventions to improve patient well-being, and evaluate progress.

Key Responsibilities
  • Evaluate patient functional status, body systems, and physical therapy needs, consulting with physicians to develop therapy plans of care.
  • Provide services within the scope of practice as defined by state laws governing physical therapy, adhering to the plan of care and collaborating with the healthcare team.
  • Observe and report patient responses to treatment and changes in condition to nurse supervisors and physicians.
  • Treat patients to relieve pain, restore function, and maintain maximum functioning using accepted standards of practice and evidence-based techniques.
  • Instruct and inform healthcare team members, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in Connecticut
  • Current CPR certification
  • Maintain a current driver's license and vehicle insurance, and have access to a dependable vehicle

We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus benefits including holiday pay, medical, dental, vision, 401k retirement plan, continuing education opportunities, and employee assistance programs.
