About This Opportunity

FeldCare Connects is seeking a highly motivated and skilled Spanish-speaking Occupational Therapist to deliver exceptional care and services to adults of all ages in the San Jacinto area.

Job Summary

This is a unique opportunity for an Occupational Therapist who is passionate about working with diverse populations and delivering high-quality patient care. As a home health therapist, you will work independently to assess, evaluate, and develop treatment plans that meet the individual needs of patients.

Key Responsibilities:
  • Evaluate patient care needs and develop personalized treatment plans to improve functionality, safety, and overall well-being.
  • Deliver one-on-one therapy sessions, lead group activities, and educate patients, families, and caregivers on therapy techniques and strategies.
  • Maintain accurate records of patient progress, communicate effectively with healthcare teams, and ensure seamless coordination of care.
  • Work collaboratively with other healthcare professionals to provide comprehensive care and support.
Required Qualifications:
  • Current Occupational Therapist license and registration in the state of practice.
  • Completion of an accredited Occupational Therapy program.
  • Ability to work independently and as part of a team.
  • Bilingual skills are a plus.
