Hospice Care Coordinator

We are seeking a compassionate and organized Hospice Transition Care Coordinator to join our dedicated team at Varco Home Health and Hospice. The ideal candidate will be responsible for coordinating and managing hospice care services for patients and their families.
Key Responsibilities- Assessment and Planning: Identify and assess patients for needed services, including medical, emotional, and spiritual support.
- Liaison and Communication: Act as a liaison between patients, families, healthcare providers, and support services to ensure seamless care coordination.
- Compliance and Quality: Ensure compliance with all regulatory requirements and standards of care, maintaining accurate and up-to-date patient records and documentation.
- Patient Education and Support: Provide education and support to patients and families regarding hospice care and services, facilitating communication and collaboration among the multidisciplinary care team.
- Crisis Intervention and Support: Provide crisis intervention and support during emergencies, participating in team meetings and case conferences to discuss patient care.
- Staff Development and Training: Provide training and support to hospice staff and volunteers, staying current with best practices and advancements in hospice care.
- Active LVN License: Must have an active LVN license.
- Hospice Experience: Minimum of 2 years of experience in hospice or palliative care.
- Strong Understanding of Hospice Care: Strong understanding of hospice care principles and practices.
- Excellent Communication Skills: Excellent communication and interpersonal skills, with the ability to work collaboratively with all types of patients and families.
- Organizational and Time Management Skills: Strong organizational and time management skills, with the ability to adapt as needed to different environments.
- Compassionate and Empathetic Approach: Compassionate and empathetic approach to patient care, with the ability to handle sensitive and confidential information.
- Electronic Medical Records (EMR) Proficiency: Proficiency in electronic medical records (EMR) systems.
- Regulatory Knowledge: Knowledge of regulatory requirements and standards of care.
- Problem-Solving and Critical Thinking Skills: Strong problem-solving and critical thinking skills, with the ability to work independently and manage multiple priorities.
- Flexibility and Availability: Flexibility to work evenings, weekends, and on-call as needed, with a valid driver's license and reliable transportation.
