Job Summary

We are seeking a highly skilled Outpatient Case Manager to join our team at Regal Medical Group, Inc. in Southern California.

About the Role

The successful candidate will be responsible for assessing, planning, implementing, and evaluating care plans for high-risk members. This will involve collaborating with physicians, other healthcare practitioners, and the member to ensure that their medical, environmental, and psychosocial needs are met over the continuum of care.

Key Responsibilities
  • Clinical Assessment: Conduct thorough clinical assessments of members to determine their level of care and develop a plan to meet their needs.
  • Case Management: Develop, implement, and evaluate care plans for members, ensuring that they receive the most appropriate level of care possible.
  • Liaison Work: Serve as a liaison between administration, members, physicians, and other healthcare providers to ensure seamless communication and coordination of care.
  • Documentation: Accurately document all interactions with members, including assessment results, care plans, and progress notes.
Requirements
  • Education and Experience: A graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program is required. Current CA RN or LVN license, current CPR certification, and valid CA Driver's license are also mandatory.
  • Skills and Qualifications: 3 years of acute care or case management experience is preferred, along with typing skills of 40 words per minute with accuracy.
