Nurse Care Coordinator

Key Responsibilities- Provide clinical direction and leadership to nursing staff, ensuring seamless care delivery and patient satisfaction.
- Develop and implement evidence-based practices, promoting a culture of excellence and continuous improvement.
- Collaborate with interdisciplinary teams to coordinate patient care, ensuring timely and effective interventions.
- Assess patient needs, develop individualized plans of care, and evaluate outcomes to ensure optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams, fostering a culture of transparency and respect.
- Associate's Degree in Nursing or higher.
- One year of experience in acute care nursing, with a focus on leadership and patient care coordination.
- SC Licensed RN - Registered Nurse.
- Basic computer skills and proficiency in electronic medical records.
