We are seeking a skilled and compassionate Physical Therapist to join our team in Palmer, Massachusetts. As a Field Home Health Physical Therapist, you will be responsible for providing high-quality care to patients in their homes, promoting independence and improving overall well-being.
Key Responsibilities- Evaluate patient needs and develop personalized treatment plans to address functional status, mobility, endurance, pain, and other medical conditions.
- Perform interventions aimed at improving patient function and reducing symptoms, in collaboration with physicians and other healthcare professionals.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
You will work closely with our interdisciplinary team to provide comprehensive care that meets the unique needs of each patient. Your expertise and compassion will make a meaningful difference in the lives of our patients and their families.
Requirements- Licensure as a Physical Therapist in the state of practice is required.
- CPR certification is also mandatory.
- A reliable vehicle and valid driver's license are essential for travel to patient homes.
