Key Responsibilities:

  • Foster collaborative relationships with interdisciplinary teams to develop comprehensive care plans
  • Assess patient needs and identify resources to support post-discharge care

  • Maintain up-to-date knowledge of community resources and services

Requirements/Qualifications:

  • Minimum 2-3 years of experience in Acute Behavioral Health or related field
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
  • Strong analytical and problem-solving skills

Salary Range: $55,000 - $65,000 per year, based on experience.
