Job Summary:

Trinity Health is seeking a skilled Medical Social Worker - LMSW to join our team. As a Licensed Master Social Worker, you will provide advanced clinical social work services as a member of an inter-disciplinary team. Your primary focus will be on engaging with patients and family members in coping with hospitalization, illness, diagnosis, treatment, and life situations, including emotional, mental, and substance abuse disorders.

Responsibilities:
  • Provide specialized skills and therapeutic education for patients and support systems.
  • Identify barriers impacting optimal patient functioning.
  • Ensure patients receive appropriate resources in the most suitable setting.
  • Conduct comprehensive psychosocial assessments.
  • Collaborate with the interdisciplinary care team and take the lead on managing complex cases, providing education, mentoring, modeling, and resources as needed.
  • Act as a patient advocate, promoting autonomy and championing patients' rights to self-determination, incorporating patients' wishes regarding care and voicing patients' interests to the hospital.
  • In collaboration with the patient and interdisciplinary team, develop, implement, evaluate, and revise, as necessary, a plan for hospital and post-hospitalization care, including referrals to other disciplines (psychiatry, addiction medicine, etc.) healthcare and community organizations based on needs assessment.
  • Identify areas for dynamic intervention.
  • Utilize brief multi-modality psychotherapeutic interventions in accordance with the treatment plan.
  • Participate constructively in Patient Rounds, Unit Care Conferences, Complex Care Committee, and other patient-centered teams as necessary.
  • Serve as an advanced clinical consultant and educator to Patient-focused Care Teams.
  • Attend and document participation in inter-disciplinary teams, including self-directed work teams, hospital and departmental staff meetings, patient care meetings, and Quality Assurance and Quality Improvement activities.
  • Actively participate in Hospital, Organizational, and community-based initiatives related to enhancing patient care.
Requirements:
  • Master's degree in Social Work is required.
  • NYS LMSW or LCSW licensure is required.
  • Minimum of 1 year of experience in Medical, Psychiatric, or Healthcare setting is preferred.

  • Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
  • The pay range may also vary within the stated range based on location.
