About the Role

We are seeking a highly skilled Patient-Centered Behavioral Health Specialist to join our team as a Psychiatric Nurse Practitioner (PNP/PMNP) at OptiMindHealth. As a key member of our clinical staff, you will play a vital role in providing high-quality care to patients with various behavioral health issues.

Job Summary:

  • Conduct comprehensive assessments and develop individualized treatment plans for clients presenting with mood disorders, anxiety disorders, personality disorders, substance misuse, and other psychiatric impairments under the supervision of consulting psychiatrists.
  • Provide psychotherapy and medication management services, including on-going support and education to patients and their families.
  • Maintain accurate and detailed documentation of client interactions, service plans, progress notes, and other relevant information.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication among stakeholders.
Responsibilities
  1. Client Care and Assessment: Conduct thorough intake evaluations, provide therapy sessions, and offer medication management services tailored to each client's unique needs.
  2. Treatment Planning and Implementation: Develop and update individualized service plans, set realistic goals, and establish clear expectations for patient progress.
  3. Communication and Collaboration: Communicate effectively with clients, family members, healthcare providers, and other relevant parties to ensure cohesive care delivery.
  4. Documentation and Record-Keeping: Maintain accurate, up-to-date records of client interactions, including service plans, progress notes, and medication management details.
Qualifications and Requirements

To be successful in this role, you must possess the following qualities and credentials:

  • Licensure in Colorado as a Psychiatric Nurse Practitioner (PNP/PMNP).
  • Experience working with diverse patient populations, particularly those dealing with mental health concerns.
  • Demonstrated ability to work collaboratively within an interdisciplinary team environment.
  • Excellent communication and interpersonal skills, with a patient-centered approach to care delivery.
  • Strong analytical and problem-solving abilities, with a focus on evidence-based practices.
