Job Title: LPN Nurse

We are seeking a skilled LPN Nurse to join our team at Providence Home and Community Care. As a LPN Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Conduct home visits to assess patient needs and provide education on self-care and disease management.
  • Document patient progress and communicate with healthcare teams to ensure seamless care transitions.
Requirements:
  • Graduate of an accredited LPN/LVN school of nursing.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Oregon Licensed Practical Nurse upon hire.
  • 1 year of experience in nursing; long term care, acute care, ambulatory care or home health and hospice care.
Preferred Qualifications:
  • Experience in home health, hospice or public health.
