RN Case Manager PRN Position

We are seeking a seasoned and ambitious RN Case Manager to help us achieve our goals at North Suburban Medical Center.
Key Responsibilities- Coordinate services through the continuum of care, balancing quality patient care with cost-effective resource utilization.
- Perform concurrent clinical reviews to determine appropriateness of admission and continued need for care delivery in the acute hospital setting.
- Support collaboration and communication among the patient/family and entire healthcare team regarding the patient's plan of care and continued needs post-discharge.
- Facilitate reimbursement for patient care services through telephonic clinical reviews with claims insurance personnel.
- Identify quality and/or risk issues and report to the appropriate individual/department.
- Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license.
- Current BLS with American Heart Association or American Red Cross.
- Case Management or CPHQ Certification strongly preferred.
- Graduate of an Accredited School of Nursing; BSN desirable.
- Minimum 2 years of clinical nursing experience in the acute care setting, with prior utilization/quality/discharge planning, and/or case management experience highly preferred.
