Job Summary

We are seeking a skilled Registered Nurse to join our Care Coordination Department as a Care Coordinator RN. This role will be responsible for coordinating patient care across the continuum, ensuring seamless transitions and optimal outcomes.

Key Responsibilities
  • Perform comprehensive assessments of patients' physical, functional, and psycho-social needs.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Coordinate with third-party payers to secure necessary resources and benefits.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Massachusetts licensure as a registered nurse.
  • Three years of clinical experience in discharge planning or case management.
  • Excellent organizational and communication skills.
