Kaiser Permanente seeks an Outpatient Physical Therapist to join our multidisciplinary care team in Clackamas, Oregon. This is a challenging opportunity to make a difference in the lives of patients and families.

About the Job

The ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering high-quality patient care. As a member of our team, you will work collaboratively with other healthcare professionals to develop and implement comprehensive treatment plans that meet the unique needs of each patient.

The successful candidate will be responsible for conducting thorough evaluations, developing personalized treatment plans, and providing evidence-based interventions to patients ranging from pediatric to geriatric populations. You will also maintain accurate and timely documentation, communicate effectively with patients, families, and interdisciplinary teams, and contribute to a positive and supportive work environment.

Responsibilities
  • Evaluate patients' physical status, functional abilities, and age-specific needs to determine current levels of functioning, self-care, self-responsibility, independence, and quality of life.
  • Develop and implement individualized treatment plans that incorporate patient-centered goals, priorities, and preferences.
  • Provide evidence-based interventions, including manual therapy, exercise, education, and modalities, to address various patient needs and goals.
  • Maintain accurate and timely documentation, including progress notes, treatment plans, and discharge summaries.
  • Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and other healthcare professionals, to ensure comprehensive and coordinated care.
  • Promote patient safety, advocate for patient rights, and participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Graduate of an accredited physical therapy program.
  • Current licensure as a physical therapist in Oregon (Washington licensure accepted).
  • National Provider Identifier required at hire.
  • Basic Life Support certification within 2 months of hire.
  • Ability to lift up to 50 pounds, push or pull carts or equipment, and perform other physical demands related to the job.

We offer a competitive salary range of $83,000 - $120,000 per year, depending on experience, plus benefits, including medical, dental, and vision coverage, 401(k) retirement plan, and paid time off.
