We are seeking a dedicated and compassionate Healthcare Case Manager to join our team at Computer Aid. As a key member of our care coordination team, you will be responsible for providing community-based care management services to our clients in the state of Delaware.

Job Summary:

We are looking for a motivated and experienced Registered Nurse Case Manager to provide high-quality care coordination and management services to our clients. About the Role:

As a Healthcare Case Manager, you will be responsible for assessing the needs of our clients and developing personalized care plans to ensure they receive the best possible care. Your duties will include conducting face-to-face assessments, coordinating care across various settings, and collaborating with healthcare professionals to achieve optimal outcomes.

Responsibilities:

  • Conduct thorough assessments of client needs and develop comprehensive care plans
  • Coordinate care with healthcare providers, family members, and caregivers to ensure seamless transitions and optimal outcomes
  • Monitor client progress, identify areas for improvement, and make adjustments to care plans as needed
  • Collaborate with interdisciplinary teams to ensure that client needs are met and care is coordinated effectively
  • Document all case management activities and interventions in electronic health records
  • Ensure compliance with regulatory requirements and company policies

Requirements:

  • Current Registered Nurse license in the state of Delaware
  • 2+ years of experience in case management and discharge planning, including experience working with geriatric populations
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
  • Proficient in PC-based word processing and database documentation
  • Reliable transportation and ability to travel within assigned territory

Preferred Qualifications:

  • Certified Case Manager (CCM) certification
  • Experience working with behavioral health populations
  • Experience working with HIV/AIDS population
  • Medicare and Medicaid experience

What We Offer:

We offer a competitive salary of $65,000 - $75,000 per year, depending on experience. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Location:

This position is based in Wilmington, DE, and requires frequent travel throughout the state.
