**Job Title:** Geriatric Care Specialist
**About the Role:** We are seeking a skilled and compassionate Geriatric Care Specialist to join our team at TEEMA. As a key member of our interdisciplinary team, you will provide high-quality patient care to our elderly population, working closely with physicians, nurses, and other healthcare professionals.
**Key Responsibilities:**
• Conduct comprehensive health assessments, develop individualized care plans, and implement evidence-based treatments to optimize patient outcomes.
• Order and interpret lab tests, prescribe medications, and perform selected procedures as authorized by attending physicians.
• Collaborate with the care team to coordinate participant management, ensure seamless transitions, and promote continuity of care.
• Educate patients, families, and caregivers on disease management, medication adherence, and healthy lifestyle practices.
• Participate in provider call schedules, attend staff meetings, and contribute to interdisciplinary care planning.
**Requirements:**
• Master's degree in clinical nursing from an accredited Nurse Practitioner program.
• 3-5 years of experience in nursing, with preference for Nurse Practitioner experience.
• NP licensure in the perspective state and Family certification from a Nurse Practitioner program.
• Ability to work autonomously, think critically, and solve problems effectively.
• Excellent communication, interpersonal, and organizational skills.
**Estimated Salary:** $120,000 - $150,000 per year, depending on qualifications and experience.
