Job OverviewWe're seeking a highly skilled Case Manager to join our team. This is a full-time position that requires strong organizational skills, empathy, and the ability to work collaboratively with a diverse population.
Key Responsibilities- Client Assessment and Planning:
- Conduct comprehensive intake assessments to evaluate client needs and eligibility for services.
- Develop individualized service plans in collaboration with clients, outlining goals and available resources.
- Service Coordination:
- Connect clients with appropriate resources, programs, and services (e.g., healthcare, housing, employment, education, or financial assistance).
- Maintain active communication with service providers to ensure seamless support for clients.
- Monitoring and Follow-Up:
- Track client progress toward goals and adjust plans as necessary.
- Conduct regular check-ins with clients to provide ongoing support and address new needs or challenges.
- Advocacy and Support:
- Serve as a liaison between clients and service providers to address barriers to accessing services.
- Advocate for clients' needs within systems and agencies.
- Documentation and Reporting:
- Maintain accurate, up-to-date case files, including assessments, plans, and progress notes.
- Prepare reports and summaries as required by management or funding sources.
- Collaboration:
- Work closely with team members, community organizations, and other stakeholders to improve service delivery.
- Participate in team meetings, training sessions, and professional development opportunities.
Salary Range: $60,000 - $80,000 per year
Education: A Bachelor's degree in social work, psychology, counseling, or a related field is preferred.
