About the Role

This position plays a crucial role in ensuring the highest quality pharmacy services to patients from neonate to young adulthood at Phoenix Children's Hospital. As a Clinical Pharmacist, you will oversee the daily activities of the medication management process and provide comprehensive pharmacy services.

Key Responsibilities:

  • Review and manage medication orders for therapeutic appropriateness based on age, weight, height, laboratory values, diagnosis, and other patient and clinical information.
  • Verify medication orders accurately and timely.
  • Effectively screen for and manage clinical alerts/conditions such as allergies, interactions, therapeutic duplications, and adverse drug reactions.
  • Actively problem-solve issues with medication orders and contact prescribers to recommend alternative medication therapy when necessary.
  • Comply with the directives of the Pharmacy and Therapeutics Committee related to Formulary Management.
  • Document clinical interventions to meet or exceed established cost containment and evidence-based medication therapy goals for the department and organization.
  • Prepare and dispense sterile and non-sterile medications accurately and timely, in compliance with departmental and organizational policies and procedures.

Requirements and Qualifications

A Bachelor's degree in Pharmacy or a Doctor of Pharmacy degree is required, along with licensure as a Registered Pharmacist in the State of Arizona. The ideal candidate will have at least 1 year of experience in a pediatric setting.

Estimated Salary Range: $120,000 - $150,000 per year, depending on experience.
