Job Summary

We are seeking a skilled Hospice Case Manager to join our team at St Croix Hospice. This role is ideal for a compassionate and experienced Registered Nurse who wants to make a meaningful difference in the lives of patients and their families.

About the Role

The Hospice Case Manager will be responsible for planning, organizing, and directing hospice care for patients in southern Lake County. You will work closely with our interdisciplinary team to provide comprehensive care that addresses the physical, emotional, and spiritual needs of our patients and their loved ones.

As a Hospice Case Manager, you will have the opportunity to work 1-1 with patients, building strong relationships and delivering personalized care that makes a real impact on their lives.

Key Responsibilities
  • Patient Care:
    • Conduct initial and ongoing assessments of patient and family needs
    • Develop and implement individualized care plans that address patient goals and priorities
    • Coordinate and manage patient care services, including medication management and symptom control
    • Provide education and support to patients and families on palliative care and end-of-life issues
  • Communication and Collaboration:
    • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
    • Participate in interdisciplinary team meetings to discuss patient progress and develop care plans
    • Collaborate with community resources and services to support patient needs
  • Additional Duties:
    • Participate in on-call duties as defined by the on-call policy
    • Ensure equipment and supplies are available to meet patient needs
    • Supervise ancillary personnel and delegate responsibilities as needed
Qualifications
  • Bachelor's degree in Nursing (BSN) from an accredited program
  • Current licensure in State
  • One to two years of recent acute care experience in an institutional setting
  • One to two years of recent experience in home care preferred
