About the Role

We are seeking a skilled and compassionate nurse practitioner to join our team at Abode Care Partners. As a House Calls Nurse Practitioner, you will provide personalized medical care to seniors and patients with complex needs in various settings, including their homes and assisted living facilities.

Responsibilities
  • Deliver evidence-based medical care by assessing patient needs, developing treatment plans, and evaluating patient progress.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, and other health issues.
  • Order and interpret laboratory and diagnostic testing as necessary.
  • Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive patient plans.
  • Refer patients to other healthcare practitioners when necessary.
  • Maintain accurate and up-to-date patient medical records.
Requirements
  • Active state Nurse Practitioner license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • A minimum of five years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
  • Proficiency in medical instruments and equipment.
  • Strong knowledge base of HIPAA and OSHA regulations.
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
  • Licensed driver with valid automobile insurance.
  • Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.

Location and Salary

The position is based in Frankfort, IN, with an estimated annual salary range of $100,000-$120,000, depending on performance and experience. We offer a competitive compensation package, including mileage reimbursement and flexible scheduling, to ensure your success in this role.
