Overview

 

We are seeking a highly skilled Hospice Care Navigator to join our team at Presbyterian Healthcare Services. This role will play a crucial part in facilitating referrals from various sources and ensuring that patients receive the appropriate care.

 

Job Summary:

 

The Hospice Care Navigator will be responsible for coordinating the clinical assessment of patients, reviewing necessary information, and obtaining approvals for the correct program and location within that program. This position assumes dual accountability for Palliative Care and Hospice referrals.

 

Responsibilities:

  • Facilitate hospice and palliative care referrals from all sources
  • Obtain reviews and coordinate necessary information for patient assessments
  • Ensure patients receive the appropriate care and support

 

Requirements:

 

To be successful in this role, you will need to possess strong communication and organizational skills, as well as experience working in hospice care. A minimum of an Associate Degree or Diploma in Nursing is required, along with a New Mexico RN License and 1 year of hospice experience.
