Recovery Manager

CareStar is seeking a highly motivated and detail-oriented Recovery Manager to join our team. As a key member of our Operations department, you will play a critical role in conducting eligibility evaluations and creating person-centered service plans for individuals applying for specific programs.

Key Responsibilities:
  • Maintain knowledge of Medicaid, behavioral health systems, and community resources to ensure effective service delivery.
  • Conduct face-to-face eligibility evaluations, including the administration of the Adult Needs and Strengths Assessments, to determine program eligibility.
  • Develop, monitor, and update individualized care plans to ensure the individual's needs, preferences, health, and welfare are met.
  • Coordinate and authorize all specialized recovery services program services, including connections to needed resources such as Medicaid services, medical, social, educational, and other resources.
  • Adhere to CareStar's policies and procedures, maintaining confidentiality and adhering to the Acceptable Use Policy while using information systems.
  • Meet the minimum qualifications of a Bachelor's Degree in social work, counseling, psychology, or a related field, or a Licensed Social Worker (LSW), with a minimum of one year of post-degree experience working with individuals with diagnosed chronic conditions or a minimum of three years' post-degree experience working with individuals with severe and persistent mental illness.
