Job Title: Occupational Therapist - Skilled Nursing Facility

At Aequor, we are seeking a skilled Occupational Therapist to join our team in a Skilled Nursing Facility setting.

Job Responsibilities:

* Assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility.
* Develop and implement individualized treatment plans to help patients regain or improve their independence in daily activities.
* Provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility, using a variety of techniques and equipment.
* Educate patients and their families on adaptive techniques and the use of assistive devices to enhance safety and independence at home.
* Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care.
* Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
* Assist in the discharge planning process, providing recommendations for continued therapy, home modifications, and support services.

Requirements:

* Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility environment.
* Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team.
* Proficiency in creating and implementing individualized treatment plans based on patient needs.
* Active license in good standing.
* A minimum of 2 years of experience as an Occupational Therapist or in a similar role.
* Certification in Basic Life Support (BLS).
* Certification in Advanced Cardiovascular Life Support (ACLS).
* Experience with EPIC or similar charting systems.
