About the Position

Amergis is seeking a skilled and compassionate Hospice Care Specialist to join our team in Prince George's County. As a critical member of our hospice care team, you will provide exceptional patient care and support to terminally ill individuals, working closely with families, caregivers, and healthcare professionals.

Job Summary

The Hospice Care Specialist will be responsible for delivering comprehensive care services, including nursing care, pain management, and emotional support. You will work collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans that address physical, emotional, spiritual, and mental needs.

Key Responsibilities

* Provide direct nursing care to patients in various settings, including homes, long-term care facilities, and assisted living facilities
* Assess patient and family needs, identifying areas for improvement and implementing solutions
* Develop and implement individualized care plans, working closely with medical directors and other healthcare professionals
* Educate patients, families, and caregivers on pain management, medication use, and other essential skills
* Ensure effective communication among healthcare teams, patients, and families through regular updates and reporting

Requirements

This role requires a highly skilled and compassionate Registered Nurse (RN) with a strong background in hospice care. You should possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. Key qualifications include:

* Current Maryland or Compact RN license
* Proficiency in English language required
* Strong computer skills and proficiency in relevant software applications
* Ability to work independently and as part of a collaborative team
