At Sholom Community Alliance, we are seeking a skilled Medical Care Coordinator to join our team. This is a rewarding opportunity for an experienced Registered Nurse to lead and provide direct care to residents in a long-term care setting.

Job Responsibilities:
  • Assume responsibility for the coordination of medical care services for residents, ensuring that their physical, mental, and psychosocial well-being is met through medication management, treatment administration, and quality personal care.
  • Utilize clinical assessment skills to identify resident needs and develop individualized plans of care in collaboration with the Interdisciplinary Team (IDT).
  • Communicate effectively with healthcare professionals, families, and other stakeholders to ensure seamless care delivery and address any concerns or issues.
  • Supervise and guide nursing staff, including Licensed Practical Nurses (LPNs), Nursing Assistants (NARs), and Therapy Maintenance Assistants (TMAs), to promote a culture of excellence and accountability.
  • Participate in resident care conferences and comprehensive assessments to identify areas for improvement and implement changes as needed.
Requirements:
  • CURRENT REGISTERED NURSE LICENSE IN THE STATE OF MINNESOTA REQUIRED
  • CURRENT CPR CERTIFICATION REQUIRED
  • ONE OR MORE YEARS' EXPERIENCE IN A LONG-TERM CARE SETTING PREFERRED
Estimated Salary:

$60,000 - $80,000 per year, based on experience and qualifications.
