We are seeking an experienced Physical Therapist to join our team in Georges Mills, New Hampshire. As a Home Health Physical Therapist, you will be part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing services to clients in their homes.

Responsibilities

">
  • Perform diagnostic tests and measurements to assess client mobility, strength, balance, and safety">
  • Develop and implement individualized care plans to address client needs and goals">
  • Continuously assess and revise care plans as needed, collaborating with the nursing team as appropriate">
  • Educate and instruct clients and their representatives on rehabilitative care and activities to promote independent living">
  • Accurately document client observations, interventions, and evaluations using state-of-the-art touch pad tablets">

Requirements

">

To be successful in this role, you will need:

">
  • A current VT Physical Therapist license">
  • A minimum of one year of recent work experience as a Physical Therapist">
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association">
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred">

Compensation and Benefits

The estimated annual salary for this position is between $80,000-$110,000, depending on experience and qualifications. Benefits may include medical, dental, and life insurance, mileage reimbursement, weekly pay, direct deposit, scholarship opportunities, one-on-one training, and more.
