About the Role

We are seeking a skilled Licensed Clinical Social Worker to join our interdisciplinary team in Riverside, CA. As a key member of our team, you will work closely with patients, families, and healthcare professionals to develop and implement individualized treatment plans.

$64,000 - $90,000 per year, depending on experience and qualifications.

Job Description

This role involves assessing, counseling, and directing medically complex patients to assist with adjustment to disability, resource linkage, and behavior modification. You will work one-on-one with patients in their homes and communities, developing tailored treatment plans to promote functional goals and independence.

  • Collaborate with multidisciplinary teams to create integrated treatment plans
  • Develop and implement individualized treatment plans for patients with complex needs
  • Provide counseling and support to patients and their families
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's degree in Social Work and state licensure as an LCSW
  • Excellent planning, organization, writing, and time management skills
  • Ability to communicate effectively with families, professionals, and co-workers
