Licensed Practical Nurse

HexaQuEST Health, Inc. is seeking a skilled Licensed Practical Nurse to join our team in a subacute unit setting. The ideal candidate will have experience working in a similar environment and be able to provide high-quality patient care.
Responsibilities:
Provide direct patient care in a subacute unit setting
Collaborate with the healthcare team to develop and implement patient care plans
Administer medications and treatments as prescribed by physicians
Monitor patient vital signs and report any changes to the healthcare team
Requirements:
Current LPN licensure in the state of practice
Experience working in a subacute unit setting
Ability to work a varied schedule, including nights and weekends

Notice:
Registry personnel must bring a state or federal issued identification to the facility and wear a photo badge at all times. Agency staff must complete their primary series of COVID vaccination in order to be considered for any assignment. State specific mandated boosters are required and must be completed when eligible. Once the assignment has started, agency staff must follow the normal testing protocol of each particular center.
