MDS Coordinator

At Westhills LTD, we are seeking a skilled and dedicated MDS Coordinator to join our long-term care team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality care for our residents.
Key Responsibilities- Resident Assessment: Conduct and coordinate the development and completion of the resident assessment process in accordance with federal and state regulations, as well as company policy and procedure.
- Care Plan Development: Identify and document nursing problems, goals, and approaches, and coordinate the development of an individual Plan of Care for each resident in cooperation with the physician, Medical Director, nursing staff, and interdisciplinary team.
- Interdisciplinary Collaboration: Schedule and facilitate interdisciplinary care plan meetings, and notify staff in advance which residents will be evaluated.
- Documentation and Reporting: Review and verify the accuracy, consistency, completeness, and signatures of all Minimum Data Sets, and ensure timely electronic submission.
- Education and Experience: Registered Nurse or Licensed Practical/Vocational Nurse with required state licensure, and a minimum of three years of clinical experience in a healthcare setting, with at least two years in a long-term care setting.
- Skills and Qualifications: Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care; excellent analytical and deductive reasoning skills, organized and detailed in work performance, and computer literacy.
