VNS Health is seeking a skilled Physical Therapist to join our team in Long Island, New York.
As a Physical Therapist with VNS Health, you will have the opportunity to work independently in the community as part of an interdisciplinary care team. Your primary responsibility will be to conduct comprehensive assessments of patients, determining their needs and developing personalized treatment plans.
Key Responsibilities:
- Deliver personalized rehabilitation therapy to patients in their homes or care facilities.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
- Practice independently in the community as part of an interdisciplinary care team.
Requirements:
- Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver's license required for all areas outside the borough of Manhattan.

Pay Range per Visit:
If you are applying to the per diem per visit version of this job for all areas other than Bronx, the hiring range is as follows: $70- $75 per visit.
If you are applying to the per diem per visit version of this job for the Bronx, the hiring range is as follows: $90 - $95 per visit.
