About the Role

We are seeking a skilled Housing Case Manager to join our team at Greater Philadelphia Urban Affairs. The successful candidate will be responsible for assisting and supporting participants in achieving full compliance with the City of Philadelphia's Office of Supportive Housing Shelter Support Services program.

The Housing Case Manager will enroll, orient, and carry a caseload of up to 32 participants, providing the full range of housing case management services. This includes conducting participant assessments, scheduling appointments, making recommendations, coordinating services, compiling reports, and keeping necessary records/case notes.

Key responsibilities include:

  • Completing orientations & registrations for new Center for Hope-Carlisle intakes as part of the Case Management team.
  • Providing housing case management services to an assigned caseload of up to 38 participants.
  • Conducting participant assessments, scheduling appointments, making recommendations, coordinating services, compiling reports, and keeping necessary records/case notes.
  • Establishing and maintaining referral relationships with service providers.
  • Arranging County Assistance Office, Social Security, and other appointments to ensure timely and continued receipt of benefits.
  • Scheduling psychiatric evaluations, medical referrals, and outpatient treatment services.
  • Identifying social services resources for participants to make appropriate referrals.
  • Making appropriate referrals to educational and vocational training programs.
  • Attending weekly Team Meetings, monthly Participant Progress Review Meetings, and monthly All Staff Meetings.
About You

We are looking for a motivated and organized individual with excellent oral and written communication skills. The ideal candidate will have a Bachelor's degree in Social Work or Human Services related field and 2-3 years of case management experience. Experience working with at-risk populations is preferred.

You will be working collaboratively as part of the Case Management team to achieve our goals. We offer a competitive salary package and opportunities for professional growth and development.

Our estimated annual salary for this position is $50,000 - $60,000, depending on experience.
