At Baystate Health, we're seeking a dedicated Community Health Worker to join our team at the Mason Square Health Center in Springfield, MA. As a Community Health Worker, you'll play a vital role in providing care to our patients, helping them navigate and access health and community services while promoting healthy behaviors. You'll work closely with our experienced providers to deliver innovative and compassionate primary care to a diverse community.**Responsibilities:**

  • Provide social support and informal counseling to patients and their families
  • Facilitate communication between patients and care teams
  • Offer culturally appropriate health education and self-management support
  • Collaborate with providers, patients, and outside organizations to improve patient outcomes

**Schedule:**

  • This is a full-time position, 40 hours a week, with a regular schedule of 8:00a-4:30pm or 8:30am-5:00pm
  • No weekends or holidays

**Qualifications:**

  • High School diploma or GED
  • Community Health Worker Certification, preferred
  • Bilingual, preferred
  • 2 years of Community Based experience required
  • Valid Driver's License, required
