Job Title: Licensed Practical Nurse (LPN)

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Dependable Health Services in Tucson, Arizona.

Job Summary:

The LPN will be responsible for providing high-quality patient care, identifying patient/family needs, and providing supportive care in accordance with the attending physician's orders and the interdisciplinary group plan of care.

Responsibilities:
  • Assess patient/family needs and provide supportive care
  • Communicate with attending physicians, other staff members, and other agencies as needed
  • Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director
  • Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status
Qualifications:
  • Current licensed as an LPN/LVN in the State of Arizona
  • Some after hours and weekend coverage is a must
  • Demonstrates good verbal and written communication, and organized skills
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Possesses and maintains current BLS certification
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
  • Currently possess or able to obtain an Arizona class 1 fingerprint clearance card
  • Home Health experience a plus
  • Bilingual (English/Spanish) Strongly Preferred
