Job Title: Registered Nurse (RN) Home Health Case Manager

Alternate Solutions Health Network LLC is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to ensure optimal outcomes.

Job Summary:

We are looking for a compassionate and detail-oriented RN to provide home-based nursing care to patients in the Dayton, Trotwood, and West Carrolton areas. As a Home Health Case Manager, you will work autonomously to develop and implement individualized care plans, manage patient risk, and promote continuity of care.

Responsibilities:
  • Plan of Care: Develop and implement individualized care plans to meet the unique needs of each patient.
  • Minimize Patient Risk: Administer medication as prescribed by the physician and prioritize visits for high-risk patients to reduce re-hospitalizations.
  • Continuity of Care: Manage multi-disciplinary care and promote continuity of care through effective communication with patients, families, and healthcare providers.
  • Family Educator/Advocate: Educate patients and families on disease processes and promote self-care through teach-back methods.
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations.
Requirements:
  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting; home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
