Job Summary:

The Senior Clinical Care Coordinator will be responsible for providing non-clinical support to our clients and caregivers, including health maintenance education, healthcare navigation, and community resource referrals. The ideal candidate will possess strong communication and organizational skills, with the ability to work collaboratively with multidisciplinary teams to ensure seamless delivery of care.

Key Responsibilities:

  • Conduct regular home visits to assess client needs and provide education on health maintenance and preventative measures
  • Collaborate with caregivers to develop and implement personalized care plans, ensuring alignment with client goals and values
  • Facilitate communication between clients, caregivers, and healthcare providers to ensure seamless coordination of care
  • Provide guidance on healthcare navigation, access to community resources, and other essential support services
  • Participate in multidisciplinary team meetings to discuss client needs and develop strategies for improving care outcomes

Requirements:

  • Licensed as LPN by Massachusetts Board of Registration in Nursing
  • Minimum 1 year experience in a skilled nursing or home care setting
  • Strong communication, documentation, and organizational skills
  • Ability to travel within assigned region, valid driver's license, and reliable transportation required
