Occupational Therapist

About the Role:
We are seeking an experienced Occupational Therapist to join our Home and Community Rehabilitation program. As a key member of our team, you will provide functional rehabilitation services to individuals with complex neurological diagnoses in their natural home and community environment.
Responsibilities:
- Perform evaluations and develop treatment plans to promote greater health and independence for patients.
- Regularly re-assess effectiveness of treatment plans and communicate patient's needs and progress to the treatment team, physician, and family members.
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes.
Requirements:
- Minimum of a bachelor's degree in Occupational Therapy from an accredited program.
- Current, unrestricted license as an Occupational Therapist by state in which practicing.
- Current CPR Certification.
- A minimum of one year's work experience as an Occupational Therapist.
