Job Description: Hospital Medication Expert

As a Hospital Medication Expert at CAROMONT HEALTH, you will be responsible for developing, implementing, and maintaining high-quality pharmaceutical care programs. Your role will involve collaborating with multidisciplinary teams to provide comprehensive medication therapy management services to patients with complex medical conditions.

About the Position:

Key Responsibilities:
• Develop and implement comprehensive medication therapy management programs for patients with complex medical conditions.
• Collaborate with interdisciplinary teams to develop and implement evidence-based treatment plans that promote optimal patient outcomes.
• Conduct ongoing assessment and monitoring of patients' medication regimens to ensure safety and efficacy.
• Provide educational programs and resources to patients, families, and healthcare professionals on proper medication use and management.
• Participate in quality improvement initiatives to enhance medication use and minimize adverse events.

Qualifications:
For this role, we seek a qualified professional with a strong foundation in pharmacy practice. A minimum of two years' experience in a hospital setting, preferably in clinical pharmacy, is required. A current NC pharmacist license is mandatory, and Board Certification is highly valued. Strong teamwork, communication, and leadership skills, along with thorough knowledge of pharmacy procedures and policies, are essential for success in this role.

Estimated Salary Range: $125,000 - $155,000 per year. Our comprehensive benefits program includes health, dental, and vision insurance, 401(k) matching, and paid time off.
