Cancer Care Navigator

The Avera Medical Group Oncology and Hematology Sioux Falls is seeking a skilled Registered Nurse (RN) to fill the role of Cancer Care Navigator - Oncology Specialist. As a key member of our multidisciplinary team, you will be responsible for coordinating patient care across multiple departments, ensuring effective resource utilization, and promoting access to appropriate services.
About the RoleThis position requires collaboration with various healthcare professionals to achieve optimal clinical outcomes and foster a positive healthcare environment. As a Cancer Care Navigator, you will play a vital role in facilitating patient-centered care, addressing barriers to treatment, and ensuring seamless transitions throughout the continuum of care.
Responsibilities- Coordinate patient care plans, collaborating with oncologists, radiologists, pathologists, and other healthcare specialists as needed.
- Facilitate effective communication among patients, families, and healthcare teams to ensure timely and informed decision-making.
- Identify and address potential obstacles to treatment, such as financial constraints, social determinants of health, or lack of transportation.
- Provide emotional support and education to patients and their loved ones, empowering them to make informed decisions about their care.
- Develop and implement strategies to improve patient engagement, satisfaction, and adherence to treatment plans.
To succeed in this role, you must possess:
- A current RN license in South Dakota or eligibility for multi-state licensure.
- One to two years of experience in oncology nursing or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Basic computer skills and proficiency in electronic medical records (EMRs).
- A commitment to providing patient-centered care and advocating for the needs of individuals with cancer.
Avera Medical Group Oncology and Hematology Sioux Falls provides a competitive salary range of $32.50 - $48.75 per hour, based on experience. Additionally, we offer:
