THE POSITION: The Patient Problem Solving Specialist is an integral part of coordination efforts within the SPA 2 Coordinated Entry System (CES). This position serves as a liaison between hospitals and homeless service providers, fostering relationships between organizations and facilitating transitions of care for individuals experiencing homelessness. The Specialist will assess and refer high utilizers to ensure a service connection is made to an appropriate CES Access Site or Outreach Team. Additionally, this role will work with internal departments and community partners to ensure that reconnections are made when necessary.
KEY RESPONSIBILITIES:
* Cultivate strong, collaborative relationships with hospital partners
* Train and educate hospital staff on access to resources within CES and other relevant resources for individuals experiencing homelessness
* Serve as a liaison between hospitals and community providers
* Participate in hospital case conferencing and collaborate in problem-solving efforts for individuals experiencing homelessness
* Provide problem-solving interventions to individuals experiencing homelessness
* Provide 'on-call' technical support to hospital staff as necessary
* Assist in tracking participant's homeless service connection and re-connecting to providers as necessary
* Triage high acuity frequent hospital-utilizers and connect to CES staff as appropriate
* Refer patients experiencing homelessness to CES Access Sites for assessment and service connection
* Provide updates to homeless service providers who have participants entering hospitals
* Track metrics as assigned by supervisor
* Document all participant interactions through case notes using clinical documentation and HIPAA standards, and maintain timely, thorough, and accurate records for reporting purposes in HMIS database
