The Social Worker (BSW) plays a vital role in promoting patient-centered care by evaluating psychosocial needs and providing community service coordination for identified patient populations. As a member of a multidisciplinary health team, the BSW acts as a liaison between the patient and community resources/post-acute care providers.
Key Responsibilities- Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
- Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual.
- Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
- Participate in the interdisciplinary care planning process and perform proactive discharge planning.
- Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
- Bachelor of Science in Social Work (BSW) degree.
- One year experience in discharge planning in a healthcare setting or BSW internship.
- Proven ability to work with others, at all levels within the organization and collaborate effectively.
- Interpersonal problem-solving skills and written/oral communication skills.
