About the Job

This position offers a unique opportunity to work as a Registered Nurse (RN) Case Manager at HCA HealthONE Rose, a Magnet-designated hospital for nursing excellence by the American Nurses Credentialing Center. As an integral member of our team, you will be responsible for coordinating patient-centered care, managing resource utilization, and developing safe discharge plans in collaboration with the multidisciplinary team.

Key Responsibilities:

  • Provide case management services for inpatient and observation patients, identifying those at risk for adverse outcomes during transition.
  • Perform comprehensive assessments of patients' psychosocial, medical, and discharge needs, considering available resources.
  • Coordinate plans of care, driving discharge plans through collaboration with healthcare teams and physicians.
  • Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
  • Evaluate progression of care using evidence-based tools and approved criteria, escalating issues as necessary.
