Case Management Coordinator

**IDR Healthcare** is seeking a skilled professional to fill the role of Case Management Assistant. The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively with patients, families, healthcare providers, and external vendors. Key responsibilities include collaborating with case managers and social workers to assess patient needs, develop and implement discharge plans, and ensure a smooth transition of care. The successful candidate will also be responsible for monitoring progress, communicating updates, and escalating any barriers to meeting transition goals. Additionally, they will maintain accurate and up-to-date documentation in the electronic medical record (EMR) and provide clerical support to the case management team. **Required Skills:**

  • High school diploma or equivalent required, with at least 1 year of recent relevant experience in a healthcare setting, preferably in case management, care coordination, or discharge planning.
  • Strong oral and written communication skills, with the ability to work effectively with patients, families, healthcare providers, and external vendors.
  • Ability to work independently and manage multiple tasks in a fast-paced environment with shifting priorities.
  • Intermediate proficiency in PC and word processing skills, along with experience using electronic medical records (EMR) systems such as Allscripts.

**Compensation Details:**

$24-$27/hr. W2. NOT OPEN TO C2C.
