Job Title: Patient Care Advocate

At Serenity Healthcare, we are seeking a highly skilled and compassionate Patient Care Advocate to join our team of clinical support staff. As a Patient Care Advocate, you will play a vital role in guiding our patients in taking back their lives by helping them understand their treatment options and providing exceptional support throughout their journey.

Key Responsibilities:

  • Act as a liaison between patients and their providers to ensure seamless communication and care coordination.
  • Deliver a personalized and empathetic patient experience at our clinics, addressing their unique needs and concerns.
  • Assist patients in understanding their treatment options and develop personalized care plans to achieve optimal outcomes.
  • Consult with patients to gather pertinent information regarding their current symptoms, mental health status, and medication history.

Requirements:

  • High School Diploma or equivalent.
  • At least 2+ years of full-time work experience in a service-oriented role.
  • Strong interpersonal and communication skills, with the ability to build trust and rapport with patients and healthcare providers.
  • Excellent problem-solving and critical thinking abilities, with a focus on delivering exceptional patient care.
