Responsibilities
  1. Provide coordination of care and ensure continuous assessment of each patient's needs, implementing the interdisciplinary plan of care with precision and empathy.
  2. Collaborate with healthcare professionals to develop and coordinate individualized plans of care, addressing physical, medical, psychosocial, emotional, and spiritual needs of patients and families.
  3. Maintain accurate records and documentation, adhering to VITAS policies and standards while ensuring transparency and accountability.
  4. Promote effective communication and shift reports through written, verbal, and body language, fostering a culture of collaboration and respect.
  5. Participate in bereavement support, understanding the importance of emotional and spiritual care in end-of-life settings.
Requirements
  • Current licensure to practice nursing in the state where the VITAS program is located.
  • A minimum of two years of nursing experience in hospice, home health, or community health, with expertise in case management and supervision.
  • Ability to sit, stand, and walk for extended periods, with occasional lifting and transferring of patients as required.
