">The Senior Case Management Professional RN will play a crucial role in coordinating patient care, facilitating communication among healthcare providers, and promoting effective transitional care management and utilization management.

Collaborate with patients, caregivers, nurses, social workers, physicians, payers, support staff, and other practitioners to facilitate effective case management.
">Foster open communication and coordination among healthcare providers to ensure seamless transitions and optimal patient outcomes.
">Utilize evidence-based practices and guidelines to inform case management decisions and promote high-quality care.
">Develop and implement individualized care plans that address patients' unique needs and goals.
">Maintain accurate and timely documentation of patient interactions, progress, and outcomes.
">Participate in quality improvement initiatives to enhance case management processes and patient satisfaction.
">Collaborate with multidisciplinary teams to address complex patient cases and develop solutions.

Requirements:

Bachelor's Degree in Nursing or equivalent experience.
">All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree in Nursing or higher within 3 years of hire.
">2 years of experience in Healthcare related field (Acute, Ambulatory, Post-Acute, etc.).
">2 years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager.

Preferred Qualifications:

Master's Degree in Nursing or Healthcare related field.
">Certification in Case Management (ACM or CCM).
