Geriatric Nursing Care Manager

We are seeking a highly skilled LPN Med Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for delivering high-quality patient care and services in a safe and organized manner.
Responsibilities- Medication Administration:
- Administer medications and treatments to residents in a safe and timely manner.
- Review and document medication updates from the Resident Care Director or Wellness Nurse.
- Ensure that medications are passed according to schedule and document any refusals or missed doses.
- Maintain confidentiality of resident information, including medication records.
- Report any resident concerns to the RCD or Wellness Nurse.
- Resident Care:
- Notify the RCD of any resident and/or family concerns through the LEAD process.
- Attend and actively participate in daily Cross Over meetings facilitated by the LCM.
- Conduct Service and Health Updates as directed by the RCD.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Transcribe orders as needed.
- Risk Management and General Safety:
- Partner with the community team to ensure compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements.
- Practice safety procedures at all times, including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
- Report any accidents or incidents immediately.
- Identify and report any unsafe or hazardous conditions/equipment.
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential.
- CPR and First Aid certifications.
- Ability to make choices and decisions in the best interest of residents.
- Ability to remain calm in difficult situations.
- Possess excellent written and verbal communication skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
