Job Summary

We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Manager, you will play a critical role in ensuring the highest quality of care and services for our residents. Your primary responsibility will be to administer medications and treatments in a safe and organized manner, while also providing exceptional resident care and promoting a positive and supportive community environment.

Key Responsibilities
  • Administer medications and treatments to residents in a safe and organized manner, adhering to established protocols and procedures.
  • Conduct daily medication reviews and updates, ensuring that all medications are properly documented and administered.
  • Collaborate with the Resident Care Director and Wellness Nurse to ensure that all medication-related concerns are addressed and resolved.
  • Develop and maintain accurate and up-to-date medication records, including medication administration records and resident medication profiles.
  • Participate in daily Cross Over meetings to discuss resident care and medication-related concerns.
  • Conduct Service and Health Updates as directed by the Resident Care Director.
  • Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
  • Transcribe orders and maintain accurate and up-to-date records.
Requirements
  • LPN, LVN, or state/province specific licensed nurse credential.
  • Maintain certifications as required by Sunrise and state/provincial regulations.
  • Previous experience working with seniors preferred.
  • Ability to make choices and decisions and act in the resident's best interest.
  • Ability to react and remain calm in difficult situations.
  • Ability to handle multiple priorities.
  • Possess written and verbal skills for effective communication and level of understanding.
  • Demonstrates good judgment, problem-solving, and decision-making skills.
