Job Description

A leading health services network in New York City is seeking a skilled Registered Nurse to join their team as a Geriatric Care Specialist in Queens. This role offers an exciting opportunity to make a positive impact on the lives of seniors and their families.

About the Role

The successful candidate will be responsible for guiding prospective participants through the clinical onboarding process, ensuring a seamless transition into the network's care delivery model.

Key Responsibilities
  • Educate prospective participants on plan benefits, services, and care delivery model
  • Assist with administrative functions, including tracking and reporting of daily activities
  • Work closely with the intake operational team regarding in-home or virtual assessment appointments
  • Conduct pre-enrollment UAS and determine eligibility of prospective participants
  • Develop service plans and obtain enrollment agreement documentation during home visits or other agreed-upon settings
  • Advise on operational process improvements to enhance enrollment turnaround, scheduling logistics, and engagement strategies
  • Document all interactions and outcomes in the designated clinical management system
  • Regularly review key performance metrics and analytics to improve process flows and quality of work
  • Support new staff with onboarding and ensure a collaborative and efficient transition to the Clinical Intake Team
Requirements
  • Prior experience in MLTC, LTTHCP, and/or Managed Care programs
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Patient-oriented approach
  • Excellent communication and interpersonal skills
  • Strong attention to detail and organizational skills
Benefits and Salary

We offer a competitive salary range of $85,000 - $105,000 per year, depending on experience, along with a comprehensive benefits package and opportunities for professional growth and development.
